Overtake / Accelerate Quickly The Car Starts To Loose Power

2005 Galaxy 1.9tdi 123k miles. This has been happening for a while but has started to get a lot worse the last couple of weeks, it's been suffering from a loss of power, as when I try to overtake or accelerate quickly the car starts to loose power as if its running out of diesel for a few seconds then picks up until desired speed is reached, happening in all gears, any ideas.

Most likely some sort of turbo issue - split hose, sticky vanes etc. If you have vcds scan for fault codes, if any come up try searching for previous issues.
